Your child will attend school in the Alpine School District (801) 756-8400 if you are moving to the following cities:

·Alpine

·American Fork

·Cedar Fort

·Cedar Hills

·Cedar Valley

·Eagle Mountain

·Highland

·Lehi

·Lindon

·Orem

·Pleasant Grove

·Saratoga Springs

·Vineyard

In order to enroll your child in the Alpine School District you must bring your child to the registration office of the school your child will be attending with copies of your child’s:

·Birth certificate

·Immunization record

·Social security card (optional, but preferred)

·Proof of residence (a utility bill, a renter’s agreement or any document that can prove that you live within the district boundaries will do)

If your child is high school or middle school age, you must also provide a withdrawal slip from the last school attended, as well as your child’s unofficial transcripts from that school.

Your child will attend school in the Juab School District (435) 623-1940 if you are moving to:

·Levan

·Mona

·Nephi

Juab School District has four schools: one high school, one middle school and two elementary schools. All but one elementary school is located in Nephi. In order to enroll your child in the Juab School District you will need:

·Verification of your child’s birth date

·A copy of your child’ immunization records

·A signed release allowing the district to request your child’s records from the previous school

Your child will attend school in the Nebo School District (801) 798-4000 if you are moving to the following cities:

·Elk Ridge

·Genola

·Goshen

·Mapleton

·Payson

·Salem

·Santaquin

·Spanish Fork

·Springville

·Woodland Hills

In order to enroll your child in the Nebo School District, you will need to call the school that your child will be attending for specific enrollment information. You can find out what school your child will be attending by call the Nebo School District office. All schools in the district require the following:

·A copy of your child’s immunization records

·Verification of your child’s birth date

Your child will attend school in the North Sanpete School District (435) 462-2485 if you are moving to the following cities:

·Fairview

·Fountain Green

·Moroni

·Mt. Pleasant

·Spring City

In order to enroll your child in the North Sanpete School District you will need to provide the school with:

·A copy of your child’s birth certificate

·A copy of your child’s immunization records

·A signed release allowing North Sanpete School District to request your child’s records from the previous school district

Your child will attend school in the Provo School District (801) 374-4800 if you are moving to:

·Provo

In order to enroll your child in school, call the school district and find out what school your child will be attending. You will need to provide the school with:

·A copy of your child’s immunization

·A copy of your child’s birth certificate

·Proof of your new address (such as a utility bill, renter’s agreement or property report)

·If your family has been living outside the United States for the last six months or more, you will need to provide proof that your child was tested for Tuberculosis upon your return to the United States.

School boundaries are set, but you can choose to enroll your child in a school outside your boundaries (within the district) as long as the school has room for out-of-boundaries students.

Your child will attend school in the South Sanpete School District (435) 835-2261 if you are moving to the following cities:

·Axtell

·Centerfield

·Ephraim

·Fayette

·Gunnison

·Manti

·Mayfield

·Sterling

In order for you to enroll your child in a South Sanpete School you will need to sign a release form giving South Sanpete School District permission to request you child’s records from the previous school.

Your child will attend Tintic School District (435) 433-6939 if you are moving to:

·Eureka or the surrounding West Desert area

Tintic School District has an elementary school and a high school in Eureka, as well as three other schools in the surrounding area. In order to enroll your child in Tintic School District, you will need:

·If your child is in kindergarten, verification of your child’s birth date (this may already be on file at your child’s previous school)

·A copy of your child’s immunization records

·Signed release allowing Tintic to request your child’s records from the previous school
